About the role
This is an engaging and rewarding opportunity to join our experienced Hounslow team. We provide personalised support and information to people diagnosed with dementia and their carers, helping them to maintain their independence.
You will;

  • Improve people’s sense of well-being and control of their lives.
  • Assist with the identification of needs, providing information and access to relevant services.
  • Accurately record your work on our database information system
  • Visit clients in their own homes, so be able to travel independently across the borough.
  • Promote our service and build relationships with a range of health and social care professionals.
About you
You will need to be self-motivated and resilient, an empathic team player who pays attention to detail. We particularly welcome applications from people who are representative of the communities we serve in the borough.
You will have;
  • Determination to support people to maintain their independence, improve their sense of well-being and put them in more control of their lives
  • Compassionate understanding of the needs of people affected by dementia
  • Ability to work inclusively with all people affected by dementia, along with an understanding of the need for confidentiality.
  • Clear communication skills, adapting your approach to ensure understanding at all levels. Language skills would be an advantage.
  • Good IT skills and experience working with data systems.
