About the Role
This is an exciting time to join Surrey County Council's Children with Disabilities Service as we embed the Family Safeguarding Model to improve outcomes for our children. Working closely with our colleagues in Education and Health, the Children with Disabilities Service aims to safeguard and promote the welfare and development of disabled children, whilst supporting them to be cared for by their families at home. The Review Team is made up of nine Family Support Workers, an Advanced Social Work Practitioner and Team Manager. We provide families with support to manage the care of children with severe and permanent disabilities.
As a Family Support Worker you will work directly with children and their families to assess and review their needs on a six-monthly basis, identifying packages of support to assist them. You will be conducting home visits, reviewing care packages across a multidisciplinary network and presenting families' needs to request packages of support.
