Adults Social Worker

Pay rate, £30 per hour
The Talent Crowd are recruiting for an Adults Social Worker in the MCC area.
The hospital discharge to assess team support citizens through the hospital discharge processes. Our input, if required, starts in hospital and continues into the community where a full assessment will take place in an appropriate setting.
You must be willing to be flexible and able to work at a fast pace. You will need to be a confident practitioner with a good understanding of the social work role to ensure effective multi-disciplinary working.
We work flexibly throughout the whole service, this includes D2A, Crisis response, intermediate care and the hospital. Social workers will work within the multi-disciplinary team both in and out of the hospital to ensure a holistic approach to assessment.

Working Hours: 35 Hours per week
