Are you an experienced Occupational Therapist looking to make a difference in the community? We have an exciting opportunity for a Band 6 Occupational Therapist to join our dynamic team in Droitwich Spa.

• Lead and manage discharge planning for patients transitioning from hospital to home or community care.
• Provide tailored occupational therapy interventions to support patient recovery and independence.
• Work collaboratively with multidisciplinary teams to deliver person-centered care.
• Contribute to the development and implementation of care plans that optimize patient outcomes.

• Degree in Occupational Therapy with current HCPC registration.
• Proven experience in discharge planning in a healthcare setting.

• Previous experience in a community-based role.
