This role has a starting salary between £40,929 to £43,735 per annum, based on 36-hour working week.

About the role
We have a vacancy in our East Surrey Team based in Leatherhead. We are looking for a competent Social Care Practitioner to join our Allocations Team working with individuals with a Learning Disability and/or Autism in order to meet their desired outcomes. This includes completing S9 Care Act Assessments, Reviews, Support plans, s42 Safeguarding Enquiries, joint working with other professionals including Health and collaborating with other stakeholders such as carers and advocates.
